These forms are to be used for the 2008-09 school year. If you need a
form for the 2007-08 school year, please use your back button to return
to the main page and select the appropriate link.
An Oromo translation is forthcoming. If your school needs this form in a language not found here, please contact community.relations@spps.org, specifying your school and language needed.
Schools may record media permission preference in Campus using the instructions attached below. Please note that all media permission information contained in Campus will be removed in mid-August, 2008. Please not not enter any information for 2008-09 in Campus until after the start of the 2008-09 school year.
