2022-23 Supplemental Pay Form - HOURLY EMPLOYEES - The 2022-23 version is for use by hourly and/or temporary staff only.  All salaried staff, and contracted and hourly teachers use the supplemental pay e-form available through Employee Self Service.  NOTE:  To be able to electronically sign the PDF form, you must first save a copy of the form to your computer before completing it.  

It is the employee's responsibility to review their paychecks to ensure they have been paid for their hours worked.  Supplemental pay may take longer to process than regular pay because it requires additional levels of approval and/or needs to be routed for a budget code to be charged.  Paper forms received in payroll by one payday will be paid on the next payday.
