COMPLETED STUDENT ELIGIBILITY FORMS should be mailed, faxed or scanned then emailed to our program office clerk, Penny Born: penny.born@spps.org or they may be given to an American Indian Education employee at your student's school. Title VI 506
The 506 form is a federal form that helps generate funding for our American Indian students under the Title VI, formula grant program. To be eligible, a student, a student's parent/guardian, the student's biological grandparent (or any combination thereof) must be enrolled in a federally recognized tribe.
All information on this form is confidential. Only one 506 for is asked to be submitted per student, and is valid for the entirety of a student's academic career at SPPS. Please note: the 506 form DOES NOT inhibit a student's ability to participate in American Indian Education Programming.

Johnson O’Malley Program
- This form will become part of your child's school record and will NOT need to be completed every year. It will be maintained at SPPS Indian Education Office and information on the form will not be released without your written approval.
- It is the purpose of this form to authorize contracts for the education of eligible Indian students enrolled in public schools and previously private schools. This local program is operated under an educational plan, approved by the BIE, which contains educational objectives to address the needs of the eligible American Indian and Alaska Native students.
- Johnson O’Malley programs offered to American Indian and Alaska Native students vary and may include such programs as culture, language, academics and dropout prevention.
