Description: Competitive football at 3 levels. Meeting Time & Place: Season starts: 8/10/15 M-F 7:00 – 9:30am & 11:30am - 1:30pm in CHS Stadium after 9/8 - 2:30 to 5:00pm
Participation Fee: $75.00 /
with fee waiver: $40.00
If you choose to purchase a girdle, the fee is $50.00
 
All forms and fees are due by August 7, 2015 to the Athletic Office.
 

The following check list of forms and participation fee are due FRIDAY before the first scheduled day of practice.

                 M.S.H.S.L.. Parent Permit and Health

                 Questionnaire / Athletic Eligibility Statement.  Please Register ONLINE this year on the Athletic webpage.

_____ 2.   An up-to-date SPORTS physical exam within the

                 last three years. 

_____ 3.   Pay participation fee (Make your check payable

                 to Central H.S. )

 

ALL FORMS/FEES ARE DUE FRIDAY BEFORE START OF SEASON…..

Do not mail forms and the participation fee to Central H.S., they must be hand delivered to the Athletic Office (room 1116) for verification PRIOR to or on the FRIDAY BEFORE the first scheduled day of practice.
